Appointments must be arranged through a licensed physician only.  To minimize any delays, please ensure that all supporting documents are included with the request for consultation including all bloodwork, imaging, and previous surgical/endoscopy procedure notes. 

 

Prior to your consultation, an online form will be sent to your email to review your medical history and symptoms.  To optimize your experience with us, and allow us to maximize the time we can spend on managing your surgical issues, please ensure that you complete these forms. 

 

Please also ensure that you bring with you a list of all your current medications.

We aim to see urgent Colorectal and General Surgical issues within 2-3 weeks.  Less urgent cases can take up to 3-4 months, and occasionally longer.  Our office will contact you with the appointment date and time.  If you have not been contacted by our office within 1-2 weeks of when your referral has been sent, please call the office in case the referral was misplaced.    ​
